I'm sure AC will correct me if I'm wrong *won't you dear..8D*, but I think Ikkaku is pretty much the same, but Yumichika has been known to use honorifics sometimes when he is in a good mood or being...er 'cute'. If you get him pissed, I think honorifics are out the window with him though...

About the rest, I couldn't say, we haven't been that intimate with that many, except Maki-maki. And I think that the lower you go in the ranks, the more respectful they would tend to be towards the higher ups..
